Copy or download the Entry and Release forms. We must have a signed copy of each form for every competitor and Boat Captain. If you change partners or Boat Captains. We MUST have new signed forms with the changes on record. Your teams will not be allowed to fish or may be disqualified if these forms are not correct. It is YOUR responsibility to ensure we have these forms fully and completely correct on file. You can email them to us or provide a copy before take-off at any event.
